Thank you for selecting the Annual PTAC Service Agreement!

You will receive a full description of the services in your email.  If you have not receive it already, please fill in the form below and make sure you include your correct email address.

Next, we will contact you to begin scheduling your quarterly services.

All Annual Deep Clean services will be billed 2 weeks prior to service with a due date of 15 days after the service is completed.

Late payments may be charged 10% for late fee.

Please read the Agreement Terms received in your email and call if you have any questions!  The phone number is in the email.
